A tennis serve is a full-body mechanical chain. The explosive force starts at the feet, transfers through the legs, multiplies in the hips and core, travels up the spine, and unloads through the shoulder, elbow, and wrist.

When an adolescent player goes through a rapid growth spurt, their muscles and bones adapt at different rates, frequently leading to stiffness in the thoracic spine (the mid-back). If the mid-back cannot rotate or extend fully during the wind-up phase of a serve, the body has to find that missing power from somewhere else. The player unconsciously forces their shoulder to over-rotate and whips their elbow violently to complete the strike.

The elbow and wrist end up paying the price for a mechanical block located in the spine. Treating the forearm without unlocking the mid-back ensures the injury returns every single match.
